Old Fellowship Baptist Church Medical Release & Liability Waiver

As the parent or legal guardian of the minor named above, I understand that, in the event that medical intervention is required for my child while she/he is at Vacation Bible School, every attempt will be made to contact me. In the event that I or my emergency contact cannot be reached, I hereby give permission to the physician selected by the VBS leaders to secure medical treatment for my child as deemed necessary.

I understand that all reasonable safety precautions will be taken at all times by the church and its paid and volunteer staff during Vacation Bible School. I understand the possibility of unforeseen hazards and know of the inherent possibility of risk. I agree not to hold the church, its leaders, employees, or volunteer staff liable for the damages, losses, diseases, or injuries incurred by my minor child, as named herein.
